Job Description

Reporting to the Director of Nursing and Human Resources, successful candidates are responsible for: participating in the client intake process of Compassion North America, as required, preparing the Care Plan for individual clients, preparing the home and site for service delivery following the procedures of the company, daily execution of the care plan for assigned clients, proper documentation of all interventions completed, timely submission of clinical reports and other documentation for payment, serve as a client advocate within the health system, coordinate the care received by the client, connect clients to other sources of support within the community whenever required to meet the goal of optimal health and independence, successfully discharging clients or closing client caseloads, as required.

Core Competencies for this Position

The following core competencies are desirable for this role:

  • Experience supporting clients with routine activities of daily living in a home care, nursing home,long-term care home or similar organizational setting.

  • Experience conducting the interventions provided in the Complex and/or Palliative Nursing Plan , provided on this website under service plans and pricing, in the acute or tertiary care setting.

These responsibilities include human resource functions: preparing job descriptions,advertising jobs, organizing interviews, hiring staff,conducting onboarding activities, establishing remuneration and benefits,conducting staff performance evaluations, planning and conducting staff development, addressing grievances, concerns, and conflict, discharging staff, and conducting exit staff interviews. These responsibilities also include nursing and clinical functions: preparing the Care Plan for individual clients, preparing the home and site for service delivery following the procedures of the company, monitoring daily execution of the Care Plan for assigned clients, reviewing all documentation to ensure that they are complete and correct, supporting timely submission of clinical reports and other documentation for payment, serve as a client advocate within the health system, coordinate the care received by clients assigned to them, connect clients to other sources of support within the community whenever required to meet the goal of optimal health and independence, collaborating with RNs and LPNs to successfully discharge clients or close client caseloads, as required.

Specific Duties Required of this Position:

· Review service requests.

· Arrange initial intake meetings and information sessions with clients.

· Review third payor criteria, standards, rules, procedures, assessments, requirements, and determine client eligibility for service.

· Assess client needs taking into consideration comprehensive information from all sources on the capacity, impairment, and requirement for home care services.

· Develop a service level agreement with the full participation of the client and caregiver.

· Prepare a care plan that adequately addresses the health condition, preferences, social and economic condition of the client, and any other relevant issues to support the client in achieving optimal health and independence.

· Coordinate service delivery to ensure uninterrupted, timely, reliable, and high-quality service to clients to help them meet the goals of optimal health and independence.

· Routinely evaluate the care plan and adapt the services and tasks provided to each client to ensure that the care plan is responsive to significant changes in the client’s condition.

· Evaluate, discharge and close a client file, when the client no longer needs home care services based on criteria, rules and standards. This may happen when the client is no longer eligible for services, the client withdraws their consent, or client is no longer available.

· Serve as a client advocate, helping clients to navigate the healthcare continuum-- providing information, education, and communication-- intervening on behalf of client’s interest as part of the care team, whenever necessary, and connecting clients to health and social support systems in the community that clients require to meet their goal of optimal health, independence, and reduce the financial burden of illness.

· Providing information, education, and communication services to clients adapting to change as a result of their illness, helping them to make the right choices that promote their health and well-being.

· Counseling clients and families adapting to change, or going through a crisis, and having difficulty to cope, make decisions or solve problems as a result of the ongoing illness of a client, or other social situation that affects the health and well-being of the client.

· Provide support and assistance to substitute decision makers and other caregivers that must act on behalf of a client that is deemed as no longer capable.

· Develop appropriate strategies to serve clients that refuse to accept home care services, but live in at-risk communities.

· Closely monitor high risk clients that require 24/7 care: including clients whose medical condition can change suddenly and become highly unstable, clients with cognitive impairment and inadequate support systems, clients that may be susceptible to neglect and self-neglect, caregivers at risk of burnout, and clients that may need crisis placement to another level of care.

· Providing human resources support services, human resource planning, advertising, collecting applications, organizing interviews, hiring staff, participating in staff development activities, conducting exit interviews, and any other human resources roles that are assigned to Care Coordinators by the Director of Nursing and Human Resources.

Technical Qualifications and Experience

  • Registered Nurse or Licensed Practical Nurse.

  • BSn or Masters in nursing or public health preferred.

  • Experience performing care coordination and human resources function for a large organization or government agency in USA or internationally is an asset.

  • Experience training nurses in USA or internationally is an asset.

  • Experience developing policies for recruiting nurses in USA or internationally is an asset.
